Confirmation Page Error

I’ve created a form which seems to work really well with one exception, the confirmation page results in an error being presented the individual who completes the form. The form actually logs all of the information, but it looks like an error to the person entering the data. The page results in a message that (form ‘mamse_2013’ not found).
No confirmation is presented, but the form does enter the information into the database.
Any help is greatly appreciated. If I could just get the user presented with a summary, this would be perfect.
