Confirmation message copies from form!

    when a form is submitted the confirmation message appears with the title of the original form, for example:

    Please fill out this form to contact us:
    Name:
    Last name:
    ….

    Once the customer clicks Submit the message he gets is:

    Please fill out this form to contact us:
    Thank you for contacting us, we’ll be with you shortly.

    what should i do to prevent “Please fill out this form to contact us:” to appear in the confirmation message?
