Confirm Before Cancellation?

Hi,
First of all thanks for the plugin.
Can you please add a feature where after clicking on the ‘Send Cancel Request’ button, the user is asked to confirm Yes/No. And If Yes, Specify a reason?
It’ll be great if the user is redirected to a new page for both of the things at once?
