Conditional to show or hide multiple fields

I have a form where i ask for certain information at the top (name, address, phone). There’s a check box underneath that, when checked, will hide a second set of name, address, phone input fields.
What is the step by step to create the conditions where
1. if the check box (parent) is checked, it hides myfield10, myfield11 and myfield12?
