conditional product variant

i want to create conditional product variant if my customer select custom size (your) so popup or and other additional field open on selected selection
1 select color
2 select size small or large or medium or custom
if customer select custom
new 5 field open length shoulder

can any one help in this what i do
any other free pulgin or any other option its urgent
