Conditional Logic on the same lables/fields

My conditional logic appears to be not working on multiple instances of labels/fields with the same name. How to address this?

If you see the form, you can see all Label Names as “Choose Frequency”. But on the backend they’re actually Choose Frequency1, Choose Frequency2, Choose Frequency3, Choose Frequency4, Choose Frequency5 and so on.. I just renamed them using javascript. How to prevent that?
