Conditional Form

I want to know if I develop a form and put on 2 3 checkboxes(radiobox most probably as in condition) and bind 3 mails to each i-e now to make it work as conditional if a user selects first checkbox and fills entire form so I want to make that entire info to be sent to the email that was bind to that checkbox user selected at first and if user selects the second checkbox i-e named support department then the info should be sent to support department mail binded to that checkbox is it possible to do this whole stuff using mail chimp form? If we can do via some other forms etc do let me know thanks.
