Conditional Fields for Contact Form 7 not working

I just downloaded Conditional Fields for Contact Form 7 but ran into issues where once I put in the starting [group] tag it hides everything within the form thereafter, it doesn’t just hide what’s inside the [group][/group] but everything in the form, including the submit button, almost as though there’s no end tag.

So rather than working on my own form, I created a new form and copied and pasted everything from the demo on:

and added the correct conditional fields on the relevant tab. I get the exact same issue on the this as well, here’s a link to my test:

Although I select a field, it doesn’t display the fields as it should.

I usually use gravity forms, but the purchased theme came bundled with contact form 7 so the styling is all ready set up, so I’m hoping there is an answer to this.
