Condition to Date picker

Date picker in Hustle does not have such advanced settings. Such a solution is only available in our Forminator plugin:

In the end, you can create a form in Forminator and later on, use the Forminator form shortcode in the Hustle popup.

Can you please guide me on a few more things like:
1) Is there no way to add radio or checkbox, dropdown to Hustle pro popup?
2) How do I change the background of whole date picker from transparent to solid?
3) The most important- After adding forminator form short code to hustle pop up, I want to keep whole of form towards right and an image on the left. Is it possible to do so?
Right now I have added my image as a background image on the left and hustle form on right.
