Completely Broken Display

My guess is its either a theme/css conflict or a plugin/CSS conflict but I cannot get this to display properly.

For example there horizontal view does not work on any element, when I drag the name/phone from advanced to the form the first name is on one line, last name another, area code another, phone another. They are not floating next to each other as they should.
