Competition Entry Popup with T&C's – Any plugin?

  • Hi Guys,

    Im looking for a free plugin that has a popup window that asks for your name, address, telephone and email. Also displays T&C’s with a check box.

    I am offering a few competitions so it would be ideal to have a plugin that allows multiple contact forms. So I know what each entry is for, they competitions are just for data collection so no questions etc.

    Hope someone can help
