WordPress 5.4.2 and PHP 7.2.34
WPForms will absolutely work with these versions of WordPress and PHP. WPForms requires a WordPress version of at least 4.9 and has been tested up to version 5.7.2. While with PHP, WPForms should work with any version 5.5 or higher.
WooCommerce 5.1
I apologize, but we don’t support any integration with WooCommerce, mainly because they’re heavily integrated with their own forms systems.
If you need forms to work within WooCommerce, I’d suggest contacting the Woo team directly (https://woocommerce.com/contact-us/) to be sure you can find an option that integrates properly with their system.

Multisite
Yes, all of our licenses will work on a multisite WordPress install. Each subsite where you choose to use WPForms will count separately towards your license’s site limit, so be sure to keep that in mind when deciding which license level you’d like.
In case it helps, here are the steps we recommend for activation on a subsite:
1) Install WPForms at a network level
2) Activate WPForms on your subsite
3) Verify license key within that subsite (under WPForms > Settings)
