Comments Pre-Populating With Previous Commenter Information

I am completely stumped about a serious privacy issue that I’ve been having for the last couple of weeks. Most of the time, when someone goes to leave a comment on a post on my site, it is pre-populating with the name/email/url from the last commenter.
The most frustrating issue is that it doesn’t seem to happen all the time. The best I can figure it may be tied to only when a specific person leaves a comment. (These are not users in WP, they are all just filling out info when they leave a comment.)

If anyone can help, I will be greatly appreciative.
