Comments box auto fill (not required!)

Hi – very new to this but have managed to get my blog/site up and running…
On one page I want visitors to be able to comment (its the absent healing page so only lists of names)
Have created the page but the contact box at the bottom (name and email) has my details in, which may confuse people – how to do I get rid of this /stop it happening pls?
