comment not working

can someone just check and see what can be done.

ok now i can use comment but this is not the way i wanted it rather i will prefer to have in different format like name comment.
how can i do that?

ok actually if you want to comment you will see single box.just to leave the comment.
I dont want like this.
i want to have 3 boxes
one for name
one for email
one for comment.
all three are needed else comment will be rejected.

you have that with name and email required to be able to post comment

but i see a big box to write the comment.
