Comment Name and Website Field

How do I get the Name, Mail and Website fields to appear before above the main text entry field? All I have is the main text entry. I am pretty sure it was showing about a week ago on my blog. I’ve tried deactivating plugins I thought might have something to do with this issue and also tried other themes.

I want those who comment to be able to enter their website to attract comments.
