Comment Moderation Issues

I’ve continue to have issues with this plugin. I’ve updated every setting within WordPress – app id, enabled FBML. I’ve also gone into the app within Facebook and added moderators and closed comments so we can review them before approving. When I go to test a comment, it immediately shows up on the page but we get no notification (within FB or WordPress) and it doesn’t show up in the Review or Hidden tabs within the app.
Am I missing something?
