Comment Form Not Appearing

I recently upgraded a site, to 1.5, using a theme i built from the Gespaa. View American Soldier Comments

As a result, the comment form doesnt seem to work anymore, as the Name, Email and URI form do not appear.

It says that Im logged in as ‘ ‘. and leaves it blank.

Any ideas?
