Columns are not working

Hi,

I used this plugin trying to create multiple columns for my form and it does not seem to be working.

I’m using the mesmerize theme. You can find the screenshots of the text and grid builder as well as the contact form on the page in the following link:

I hope you can help me. I think it has something to do with the css, I don’t think it’s reading it right.
