Collecting more data at user registration

I have a client with a classified ads site. The site offers free ads to HOA members in her community. They want a way to collect the users name, address, and phone number on the registration form so they can confirm they are HOA members. Second, they want to approve or reject registration based on the address provided (this will be a manual review process…nothing automated).

Any idea of a plugin or combination of plugins that will accomplish this?
