Collecting and displaying Custom User Submitted Posts

I’ve been over dozens of entries on the forum and tried as many solutions, but I haven’t been able to do what I want. I’m creating a review site that reviews items by several criteria. My form is approximately 16 fields.

What I want:

1. I want to create a form with custom fields (text, rating 1-10, multi-select).
2. I want users to be able to fill out the form and either control submission via permission level, or via moderator approval. (either works for me).
3. I want to be able to display approved submissions on a page–and allow users to filter them by 3-6 of the internal fields. Similar to filtering a blog by tags.

Things I’ve tried:

1. I’ve built a form in “Forms Manager” to collect the data, and it works fine. I cannot figure out how to display it as a formatted post with the individual fields intact.

I can print it to a single blog post–but that does not give me the sorting capabilities that are my core requirement: allowing users to sort the library of review submissions by several of the categories.

I’ve also built the form in “Advanced Custom Fields”–I really like how the form works, but I’m at a loss as to how to build the output half.

Any direction or advice is greatly appreciated
