Collect feedback from Users / Employees

We have a need to collect some feedback from the employees, the kind of functionality I am looking at is if the employee enters his/her ID number the form should return values such as name, department, location etc. from a database / sheet / CSV stored somewhere on the server. (we have an open intranet internally so employees do not login)

Any ideas which form plugin would could help?

Also after submission we would like to validate duplicates (stop re-submission) and get some analysis of submissions eg. 10/20 employees of HR department have still not submitted the form.
