code to use form in template

Hi,
I have a form that works fine if I display in a post, but I need to add it in the template.
When I add it in the template, the text search box does not appear.
Is there another argument to add?
I have used the code listed in your FAQ page eg
$arg=array(‘id’=>102); echo get_uwpqsf_form($arg);

If I use “do_shortcode” it works correctly.
