    The output is given twice in your page. Did you accidentally paste the [feedback] shortcode twice?
    Also, there seems to be unnecessary paragraph tags inserted. Some shortcode plugin might cause this. Can you please share the list of plugins you are using?

    All I have in the “HTML” section of my page editor that contains this shortcode is this:

    <h1>MKI Post-Inspection Survey</h1>
    <h3>Tell Us About Your Experience</h3>
    [feedback]

    I dont have any extra HTML that would break the code nor do I have any extra shortcodes added.

    The plugins I am using are:

    contact form 7
    Add Link to Facebook
    Facebook
    Jetpack by WordPress.com
    Post Types Order
    Quick Page/Post Redirect DEV
    Ultimate TinyMCE
    and of course the WP Feedback & Survey Manager

    I dont see how any of these plugins would do it when they are not being run on the page I have it on. The page is set to turn off the sidebar and just be the survey. Maybe its my theme. I am using the Office responsive theme from WP Explorer. You can find it on themeforest.com or wpexpolorer.com.

    I guess this is the theme then. It would be best if you ask for their support team.

    I have this same issue – first off: great plugin man I like it but I really don’t think it’s a theme issue as it can’t be everyone’s theme causing the error. Second: Yes I think it may possibly be a plugin issue but I have deactivated all plugins and reactivated one at a time to no avail. I am keeping the plugin tho because I only have this issue when I use the feedback tab, I don’t get any error when I only use the “personal info” and “survey” tabs.

    Only happens when I include the “feedback” tab and it’s the same exact issue as nirion209 above. If I run across a fix I will post it for other users ??
