                    <div class="modal-dialog" style="width: 750px">
                        <div class="modal-content">
                            <div class="modal-header">

                            <div class="modal-body" id='wpdm-modal-body'>
                                <p class="placeholder">

    I have investigated further and this code comes from WPDM extended shortcodes PRO. This is absolutely shocking practice.
