clicking login with sms shows server error

Hello Sir, we are having issue in integrating your plugin with our website. we have created facebook app for the same. and inserted the values of app id and secret code . Also we tried adding up the scripts in the header section. but it still shows simple server error in the dialog box. no error message were displayed in console. your help will be highly appriciated. thank you.
