clean and works

I’ve tried several plugins but most of them has very bad UX and not to mention the aweful configuration part, I did settle down with one plugin but turned out the email addresses weren’t being delivered to my email account. BUT This plugins works with simple design, I don’t have to search for anymore, I can rest now.
