The “Thank You” page is essentially what WooCommerce uses to change the order status from “Pending Payment” to “Completed” and while it is possible to redirect the user to another page, it isn’t advised.

That said, you could look at the following resources:

As a sidenote, have you considered getting the user to fill out the form before the payment is processed? Possibly with the use of https://www.woocommerce.com/products/product-add-ons/ or https://www.woocommerce.com/products/gravity-forms-add-ons/ (Requires Gravity Forms) ?
