Checkout field for each product

I wish to add conditional field for product which I have been able to do successfully.

My question is how do I add conditional fields for each individual product sold if it is the same product?

E.g. I am selling 2 individual tickets of same type, and I want to add fields to capture name for each ticket?

I am only able to add one conditional field even if there are multiple products of the same type sold.

Any help appreciated.
