Checkboxes with title

  • I’m building a form with WordPress’s built in contact-form shortcode. It has an option to create checkboxes in the form, but I would like to group a bunch of textboxes with a title, like such:

    TITLE WILL GO HERE.
    [] Checkbox 1
    [] Checkbox 2
    [] Checkbox 3
    …

    Right now the closest I’ve come looks like this:

    [] TITLE WILL GO HERE
    [] Checkbox 1
    …

    (the title itself has a checkbox)

    Apparently radio boxes support having a title, but I need checkboxes.
