Checkboxes appearing as a text field

    I am using a form with multiple pages, downgraded from the new version to the old version of Google forms.

    I have this checkbox in my form (second page): https://prntscr.com/g7b3ta

    It seems to be appearing as a text field on my site (a page with the shortcode embedded), with no options visible. If you enter text it gives an error and does not let you continue to the next page: https://prntscr.com/g7b42y

    I do have “Disable HTML Filtering” checked, because I wanted to hide a link that was appearing at the bottom of the form. Even if I turn it off I still get this error, though.

    Have you seen this error before?

    We were able to pinpoint the problem due to a setting in my theme. I disabled the form styling setting and was able to get things working again.
