Checkboxes and Radio Buttons Not Showing Up In Chrome

I am using your form builder on a client’s website and I noticed that the check boxes and radio buttons don’t show up in Chrome. They work fine in Firefox, but why don’t they show up in Chrome? Is there a fix for this?
