Checkbox & radio button not showing

What other plugins and theme do you use on the site?

The theme is Divi and the plugins are : maintenance (webfactory ltd’s), PDFjs viewer, PuzzleMe and WP Mail Logging.

Try switching to the default theme, deactivating all plugins excluding Contact Form 7, and clearing all cache data.

Why you are advised to deactivate plugins and switch to the default theme.

Thanks! Divi was conflicting so I contacted the support and they helped. For anyone having the same issue in the future, this how they fixed it:

-webkit-appearance: checkbox !important;
-O-appearance: checkbox !important;
-moz-appearance: checkbox !important;
display: inline-block !important;
}

Adding this to the custom CSS section.
