Checkbox questions not displaying all answers

Hello,
I have a survey that uses check box questions for users to enter multiple answers that apply. When submitted and received, these responses only display one answer which seems to be random (sometimes it gives the first checked answer, other questions it will give the last answer).

Any help is appreciated.
