I could reproduce the issue even on desktop using the Google Chrome browser — screenshot.

However, I tried on my dev site, with a default WordPress theme and a page from the block editor, and I had no issues using iPad with Safari — screenshot.

With this, I recommend adding your form on a new page using the block editor, and checking if the issue continues. If it does not, this might be a conflict with your template.

If the issue continues, this might be a conflict with your theme. To troubleshoot this, please try the steps described in our detailed guide on how to test for plugin/theme conflicts.

A new page with the WP Forms Build Options is created, with the Block editor it looks good, but when I visit the page I get the same error:

Checking for Theme Conflicts

I have activated the theme “Twenty Twenty-FourVersion: 1.0” and the bug is gone. It’s an issue with my theme.
