Checkbox for “Note” feature not displaying correctly

    I have the checkbox and note enabled on my MailOptin form, but when people check the checkbox to agree to my terms, nothing appears in the checkbox. People can’t tell if the checkbox is checked or not. I’ve tried on several different computers and browsers but the checkbox does not seem to be displaying/working correctly.

    Any thoughts?
