Checkbox field empty on edit screen

    This has happened twice now in the latest version – creating a checkbox field – it appears just fine in the form display but when you have to edit it – the edit screen is blank. The options don’t even load. (took a screenshot) — I’m having to completely recreate that field again.

    also — cannot duplicate the field. Clicking that option does nothing. (and the name field just did the same thing – went to “edit” screen and it loaded empty.)
