CF7 v5.4 not working on WP 5.6.2

    I just upgraded WordPress to 5.6.2 and CF7 to v5.4.
    Post upgrade, Contact Form in modal is not working — REQUEST QUOTE button in header menu & above footer.

    Console shows error 404 resource not found.
    Not sure what’s gone wrong.

    Any help is appreciated.
