CF7 Unable to Send All Form Field Data

Hello, my cf7 form is able to send collected form data alright to my email address. But the problem is, some of the field data are not sent at all. For instance I have created a “Service Request” form, where I have fields such as name, email, phone, service type etc… It’s only able to send data in the name field, but unable to send the rest.

Please what could I be doing wrong?
