Category problem!

I am trying to set up a site which allows users to review local stores, this means I will need categories as follows.

• Country
• County
• Name and Shame
• Name and Praise

I would like to allow users to just post in either of the last two and nowhere else, if doing this however it just shows as Name and Shame or Name and Praise in the category list without the Country or County showing, is there an easier way to set up the categories?

Also, when you click on a category I would like just a list of clickable posts with no post text just the heading.
