    Go to Structural Settings under Pricing Table Settings. At the bottom, you will get option to Enable Caption Column.

    does it work with multisites?

    or do I need to activate per subsite?

    The plugin doesn’t have support for multisites and I didn’t check this plugin in multisites. You can check and let us know about it.
