captcha only shows part of the time

Captcha shows up sometimes and sometimes not. Checking my 404 logs shows that wp-content?/uploads?/wpcf7_captcha?/537467125.png is missing when it is not showing up on the form. This happens several times a day. In Firefox it is just blank and in Chrome it shows a missing image icon.
