Captain Form

Hi we are using captain for the application of volunteers and unfortunately, all of a sudden the captain forms on the mobile does not hold the information. when someone types the info disappear. I have tried safari, chrome, duck duck go and problem persist on iPhone and iPads so far confirmed. I been using these from few years and never come across this issue. Please,help….. it is very urgent for us
