• After upgrading from 3.7 to 4.2.2, users now get the following error message after clicking the Add Media button and selecting a file:

An error occurred in the upload. Please try again later.

I have tried deactivating the plugins, and that seemed to have no effect.
Changing the theme to Twenty Fifteen, however, allows normal media uploads. So the problem appears to be with the theme. It’s a custom theme, so I need to update it somehow to work with 4.2.2.

What in a theme might interfere with the Add Media feature?

Yes – your theme seems to be the issue.
