    The strange thing is, it doesn’t happen all the time. I sometimes can try it for 6-7 times before the error pops up.

    No, nothing jumps out at me. I took a look at the page with Firefox Dev Tools (my favorite tool Firebug seems to be deprecated – bummer). There are a bunch of odd font load errors reported but I didn’t see any Javascript errors.

    One thing to try is to use the permalink for the form instead of the page you put the form shortcode on. That should remove some of the WordPress loop from coming into play.

    One other thing I noticed is your theme defines the “error-message” CSS class which is why you see the error about email address in red. You can get around that by using a CSS prefix when you set up the Google Form in WordPress.
