Cant see check box or radio options

    I have been trying to get the check box and radio options working on my page (located at the bottom of the page.

    I have also tried the below as custom CSS but doesnt work:

    div.wpforms-container-full .wpforms-form input[type=radio] {
-webkit-appearance: radio !important;
	-moz-appearance: radio !important;
	appearance: radio !important;
}

    When I visited the website, I see shortcodes showing on the front end. When you have some time, could you please check if the plugin is activated or the form that was embedded on the page still there?
