Can't replicate album extend display format

  • WordPress up to date? Yes V3.5
    NextGEN up to date? Yes V1.9.10

    (please confirm the following, with your server host tech) :
    MySQL up to date (5.2.4)? MYSQL Version : 5.5.23-55
    PHP up to date (5.0)? PHP Version : 5.2.17
    The mod_rewrite Apache module activated?
    PHP Safe Mode OFF? PHP Safe Mode : Off

    and if you’re willing to share:
    Who are you hosted with? HostGator
    What theme are you running? Defaulted to Twenty Eleven for debugging but MystiqueR3 before that
    Which plugins do you have installed and activated? Deactivated all but NextGen
    Does this error still occur if you switch your theme to the WP default, Yes
    and deactivate all plugins except NextGEN Gallery? (*be sure to clear your browser cache and reload your site after deactivating)

    So, I am trying to replicate the album extend view which I successfully implemented here:

    I have an album comprised of 7 galleries with a total of 29 images.
    I carefully manually re-uploaded all 29 images using the exact same gallery structure and tags

    However, I cannot get the album to display in extend mode on the second domain here

    I am using this simple line of code in both cases

    [album id=x template=extend]

    x = 1 for LocalSearchConsulting.org domain
    x = 3 for CustomImageAds.com domain

    I have spent all day trying to diagnose this. Any advice appreciated
