yes. there is a folder and it contains css, docs, js, images, languages folders.

I’m not sure. there’s an “includes” folder which contains various php files… including script.php and ajax.php. I don’t understand these…

no. only an unrelated php code, text and categories widgets appear there

I’ve looked at it but nothing seems to refer to the banner, only to the larger form below the banner. But, then again, it’s less familiar to me than mandarin chinese…

In the source code, the banner is within the div .form_description, which doesn’t appear anywhere within the machform.php file.

Yeah it is really hard to trouble shoot this over the interwebs, could you possibly post a pastebin ( where it says syntax highlighting please select php! ) of that file, I am wondering if inside there it is importing content aswell.
