Can’t insert images from media uploader when Custom Field Template is active

For some reason, when this plugin is activated (v 1.08), my media uploader no longer works properly. I am still able to upload files, but when I click the “Insert Into Post” button, the uploader pane turns white and nothing happens.

As soon as I disable the plugin, the functionality comes back right away. Does anyone have any ideas?
