Can’t Go to My Forms

    When I log onto my WP Dashboard, it a message from CaptainForm saying that I got a duplicate account and ask me if I would like it to replace (not the exact word-but similar); So I choose to replace it.

    Now when I click on “My Forms” it just take me to my Account page. Please help!
